The CLN-AQuA Henderson Quality Award is presented in honour of Steve and acknowledges achievement in innovative projects and supports the growth of service improvement across the NHS.&lt;/div&gt;&lt;div style="margin: 0px 0px 10px;"&gt; The successful first prize winner will receive a £5,000 bursary to develop and spread their innovation, as well as additional support from David Fillingham, AQuA Chief Executive. Two runners up will also receive bursary prizes.&lt;/div&gt;&lt;div style="margin: 0px 0px 10px;"&gt; In particular, we are looking for clinicians who can demonstrate elements of quality in their projects and how these have led to greater productivity within their service area. The programme is designed to empower and enable Black and Minority Ethnic colleagues to reach the most senior positions in the NHS. Yvonne has championed the &lt;a href="http://clnreal.wordpress.com/"&gt;CLN REAL Network&lt;/a&gt; as it has evolved, enabling and supporting the first cohorts of Eminent and Aspiring Leaders to commence their leadership development activities. Here Yvonne shares her personal blog ……&lt;/strong&gt;&lt;/em&gt;&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;My team consists of seven people: three regional coordinators, three administrators and me. I am incredibly proud of the team and everything we have achieved together. They are amazing people and each of them brings something unique, something special to the team. Breaking Through would not be Breaking Through without them. We have been a team for just under a year but I believe we are a tight and cohesive group. Teams that are high energy are so important. Without a driven and resilient team that’s moving in the same direction it would take so much longer to hit objectives or to have any impact at all. The new NHS needs to have highly productive teams that can deliver successfully on a very difficult agenda.&lt;br /&gt;&lt;br /&gt;So what makes a great team? I believe it’s several things (and this is just my view).&lt;br /&gt;&lt;ul&gt;&lt;li&gt;Everyone understanding the direction of travel and sharing the vision – but more than that, actually wanting to go in that direction and believing it’s the right way to go.&lt;/li&gt;&lt;li&gt;Having difference in the team. Every member of my team brings something uniquely different to the party and it is that diversity that makes a difference.&lt;/li&gt;&lt;li&gt;The ability to work hard and go over and beyond the call of duty. There isn’t a single member of the team who won’t go the extra mile.&lt;/li&gt;&lt;li&gt;Genuinely caring for each other and the team itself.&lt;/li&gt;&lt;li&gt;Flexibility, tolerance, openness and honesty.&lt;/li&gt;&lt;li&gt;And of course good, high quality, confident leadership.&lt;/li&gt;&lt;/ul&gt;I’m sure you’ll agree that what I’ve described is the crème de la crème of teams. Let’s be honest most people are not lucky enough to work in teams like that, at least not all the time. I have worked in and led many teams in the NHS and can attest to the fact that lots of energy goes into trying to make teams function as a unit; sometimes the poor manager/leader has to deal with dissent, disagreements, dissatisfaction and disobedience or all of these at the same time. It is hard work moulding and motivating a team to be successful, sometimes the skills it takes to do this go unrecognised or unappreciated.&lt;br /&gt;&lt;br /&gt;So you can imagine how grateful and thankful I am that I have such a high quality productive team. The interesting thing with a team that is so cohesive is what happens when a new member joins. If the new member assimilates and becomes part of the team quickly, understanding the culture, the do’s and the don’ts, what’s acceptable and what’s not, then all is well with the world and productivity remains high. However, if the new member does not fit and does not want to integrate, or worse than that the team does not want to welcome the new member in, then the dynamic can be very different.&lt;br /&gt;&lt;br /&gt;On reflection, this is where the quality of a good leader needs to come to the fore. It is incumbent on them to work out how to help the newcomer become a fully functioning and integral part of the team. In my experience this is easier said than done but done it must be. The sum of the parts is greater and stronger than the individual units, therefore enabling the team to continue delivering whilst ‘storming and norming’ is the leader’s priority.&lt;br /&gt;&lt;br /&gt;In the coming months, many of us will be joining new teams or have new members joining our teams. Here, he gives us an update on the project and how it is progressing…&lt;/em&gt;&lt;br /&gt;&lt;br /&gt;I won the CLN award for my project to introduce remote monitoring (RM) of cardiac implantable electronic devices (CIEDs),which is a process whereby patients substitute outpatient review for safe, remote monitoring of their devices at home. Remote monitoring means that we can pre-set alerts, which ensures changes in either the device or the patient’s condition are notified to the specialist team much earlier than usual. As a result, RM is not only more efficient and convenient for patients, but is able to deliver safer and better quality patient care.In the climate of QIPP, I believe this is a key example of how innovation can improve quality and save cost simultaneously.&lt;br /&gt;&lt;br /&gt;RM often avoids the need for patients to attend conventional cardiology clinics and also enables more frequent surveillance of their pacemaker, enabling us to detect any problems at an early stage and avoid emergency hospital admissions. For example, as a result of the RM we can now detect and treat atrial fibrillation much earlier, helping to reduce the risk of stroke. For clinicians and the NHS, we can now dedicate more time to patients who need quality face-to-face attention and there has also been significant cost saving, by freeing up workforce time. We have recently held four patient groups to gain feedback on the project, and so far we have had really positive comments about how pleased they are to be able to be monitored from home, and to be able to communicate with the cardiac team as and when a problem arises. &lt;br /&gt;&lt;br /&gt;The cultural change to remote monitoring also promotes a more widespread move to managing patients NOT devices alone. At our Trust, I have led implementation of remote monitoring for patients with heart failure, a long term condition which is a significant consumer of resource, and for patients with implantable defibrillators, since December 2010. Now all pacemaker patients are recruited to remote monitoring and at over 90%, the uptake by patients demonstrates its popularity. &lt;br /&gt;&lt;br /&gt;The change in culture and behaviour for staff has also been significant. To remove face to face follow up and articulate the new telemedicine scheme required significant clinical engagement. For patients, we used support groups, expert patients and industry partners to demonstrate the quality improvement potential of RM. Clinical engagement is key to implementation but equally, managers are key to operationalising a shift from conventional OPA activity to RM. &lt;br /&gt;&lt;br /&gt;I have now started to use the bursary to enable this innovation to be spread across the West Midlands, by supporting the practical implementation of my strategy and developing monitoring systems. Regardless of whether you are a cynic - and see this as another example of increasing privatisation of the NHS -&amp;nbsp;or an enthusiast- and are stimulated by the goal to touch 3 million lives through assistive technologies for example - there is plenty in it for everyone &lt;/span&gt;&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Arial&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;It is a must read. Stopping there, however, is not an option, we have to move to must-do; unless we innovate the NHS will go bust. The Health Action Zones followed by the Modernisation Agency and more recently the National Institute for Innovation and Improvement have all tried over the last decade, and done some sterling work, but “Doing more with less and faster” and “spreading and sustaining innovation” remains a dream in the NHS.&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;The innovation Review has therefore come up with some very ambitious goals, and has plans to ensure better data, align incentives and undertake strong performance management to ensure it succeeds. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;  &lt;/span&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 0pt;"&gt;&lt;span style="font-family: &amp;quot;Arial&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;Having been around for a long&amp;nbsp;time in the NHS, although I welcome this review I am also wary, and feel that unless we do something different this attempt &lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp;&lt;/span&gt;will also go the way of its predecessors. Speaker after speaker, and all eminent ones, at the recently held NorthWest Health Innovation and Education Clusters (HIEC) conference (&lt;/span&gt;&lt;a href="http://www.hiecsnorthwest.org.uk/"&gt;&lt;span style="color: blue; font-family: Arial, Helvetica, sans-serif;"&gt;www.hiecsnorthwest.org.uk&lt;/span&gt;&lt;/a&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;) confirmed the importance of Innovation in health care given the financial challenges facing the NHS. However, whilst being motivated and feeling more confident, it was not clear to participants how to drive innovation on the necessary industrial scale. Most case studies still showed important but marginal changes. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;  &lt;/span&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 0pt;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;span style="font-family: &amp;quot;Arial&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;It seems to me that the key issue here is that of strong clinical engagement and leadership, without this progress will remain slow. In this regard I believe that the CLN is ideally placed to drive the agenda forward. Its members are amongst the most progressive and committed clinicians in the NHS and they have already shown improvements in many areas. Creating a more structured and managed approach at the CLN will speed up the process- there is nothing to lose and everything to gain by promoting the safest and most efficient practices through adoption of proven innovation. After all, how can we, as clinicians, justify the existing variations? How can we continue to condone limited uptake of proven innovation to reduce waste and prevent harm?&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/span&gt;&lt;br /&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;  &lt;/span&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 0pt;"&gt;&lt;br /&gt;&lt;/div&gt;&lt;span style="font-family: &amp;quot;Arial&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;If we do not take the lead , then we will be pushed into it. The message was reinforced very clearly by an indignant (lay) participant at the above conference who said that she was going to beat down doors and demand answers to why we were failing to do so. They ask ‘How can we remain steady and consistent however rocky things get?’ Some of the approaches discussed include ‘think options, not solutions, embrace ambiguity, avoid feeling overwhelmed and&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;learn to value not knowing’. &lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Calibri&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;A subsequent article by John drew and Helen Bevan on the 1&lt;sup&gt;st&lt;/sup&gt; December urges managers to build a culture of mutual ownership and trust rather than leading from the front.&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;The authors comment of the analysis of the NHS Top Leaders programme demonstrating that the predominant senior leadership style has been one of ‘pace setting’.&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;In response to this, the authors say ‘ As winter draws in, this leadership style is likely to be less effective in a world where success depends on collaboration.’ They go on to suggest that in the current context leaders need to develop a commitment-based strategy, based on ownership, meaning, mutual trust and understanding. &lt;/span&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;br /&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 10pt;"&gt;&lt;span style="font-family: &amp;quot;Calibri&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;As a practising clinician with an interest in clinical leadership, I am surprised that it has taken the present political and financial climate of the NHS&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;for its senior&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;leadership to realise that effective sustainable leadership of healthcare teams requires a myriad of skills, styles and approaches. Amidst the structural turbulence that we are experiencing and the pressures of the Nicholson challenge, we have rather critically forgotten about caring for our most valuable asset, the workforce. &lt;/span&gt;&lt;o:p&gt;&lt;/o:p&gt;&lt;/div&gt;&lt;span style="font-family: &amp;quot;Calibri&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;Most of us joined the NHS with a strong level of personal commitment and core values around providing good care for patients. Throughout the last two decades we have lost sight of this fact and to some degree chastised the workforce for being inefficient, resisting change and not showing good leadership. Experience and the evidence shows us that a valued workforce will go that extra mile, without a ‘dictat’ from above, and that they will have developed an in-built resilience through this process.&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;Developing ownership, mutual trust and understanding takes time, honesty and integrity and will not occur overnight despite the current environment.&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp; &lt;/span&gt;&lt;/span&gt;&lt;br /&gt;&lt;br /&gt;&lt;span style="font-family: &amp;quot;Calibri&amp;quot;,&amp;quot;sans-serif&amp;quot;;"&gt;&lt;span style="mso-spacerun: yes;"&gt;&lt;/span&gt;As clinicians within the NHS we are the fabric of the system and we will remain so, through successive governments, ministers and health policy changes. Remaining true to our core values and having intellectual honesty in all that we do and advocate as professionals will stand us, and our patients, in good stead for the short, medium and long-term future of our NHS. The John Lewis Partnership and Marks and Spencer have a reputation for the longest periods of service from their employees. Valuing both their customers and staff alike is part of their recipe for success. David has a Diploma in Personnel Management and a Masters Degree in Health Service Management and Local Government. In 1993 David was appointed CEO of the Princess Royal Hospital NHS Trust Telford and he retired from the NHS in 2003. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;  &lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 10pt;"&gt;&lt;span style="font-family: Calibri;"&gt;This summer&lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp;&lt;/span&gt;(2011)David wrote a primer on e-Health, plus a separate paper on the cost effectiveness of Telemedicine, he has also developed models of care which combine community based resources and telemedicine. He is currently writing a paper “The case for Citizen controlled Care records and Localised Care Information. Giving the Citizen Responsibility and control – a paradigm shift.”&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/div&gt;&lt;span style="font-family: Calibri;"&gt;In his latest paper he says of e-Health: &lt;span style="mso-spacerun: yes;"&gt;&amp;nbsp;&lt;/span&gt;“HMG understands this fact of life and want to exploit the change in our socio-technical behaviour patterns in order to reduce the size of or at least stabilise the tax burden in Society. We want to ensure that the patient receives the most appropriate care and further follow up, especially after a fall. &lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;  &lt;/span&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 0pt; mso-layout-grid-align: none;"&gt;&lt;span lang="EN-US" style="font-family: &amp;quot;Arial&amp;quot;,&amp;quot;sans-serif&amp;quot;; mso-bidi-font-size: 10.0pt;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;Initial work has shown variability in how telecare organisations decide whether to call 999 after receiving a pendant press for a fall, many simply have no one else that can respond to a patient on the floor who states that they are uninjured, are well and simply can’t get up&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;  &lt;/span&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 0pt; mso-layout-grid-align: none;"&gt;&lt;span style="mso-ansi-language: EN-GB;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;I have been working with Sheffield City Wide Alarms and Leeds Care Ring to ensure appropriate responses to people who use pendant alarms to call for help. New clinical decision algorithms have been developed for use in the telecare monitoring centres to support safe decision making about whether an ambulance is required. A new option to access expert advice from YAS Clinical Advisors was added to the algorithms. Results from show a reduction in direct 999 calls from the monitoring centres, improved satisfaction in monitoring centre staff, and increased use of Clinical Advisors and Emergency Care Practitioners&lt;o:p&gt;&lt;/o:p&gt;&lt;/span&gt;&lt;/span&gt;&lt;/div&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;  &lt;/span&gt;&lt;br /&gt;&lt;div class="MsoNormal" style="margin: 0cm 0cm 0pt;"&gt;&lt;span lang="EN-US" style="font-family: &amp;quot;Arial&amp;quot;,&amp;quot;sans-serif&amp;quot;; mso-bidi-font-size: 10.0pt;"&gt;&lt;span style="font-family: Arial, Helvetica, sans-serif;"&gt;We have identified that many services are not trained or equipped to handle and move people from the floor or to undertake basic first aid assessments and so readily default to the 999 service.
